Every mattress is cut to the precise dimensions you give us; the cover then adds a small amount to the finished size.
Where the bed sits in a tight recess — as it often does in VW and coachbuilt conversions — or you like a little room to tuck bedding in, trim your measurements down slightly. That part is purely personal preference.
